How to Use an EpiPen

If you or someone nearby shows signs of anaphylaxis, seek emergency medical care immediately. EpiPens are indicated for the emergency treatment of anaphylaxis and should be administered at the first sign of a severe allergic reaction. Prompt use is critical—delays can be life-threatening.

  1. Remove the EpiPen from its protective plastic case.
  2. Hold the EpiPen in your dominant hand near the gray safety release cap—without covering it.
  3. With your other hand, remove the safety release cap.
  4. Avoid touching the black tip (this is where the needle exits).
  5. Aim the black tip toward the outer thigh.
  6. Swing and firmly press the EpiPen into the thigh at a 90° angle.
  7. Hold in place for approximately 10 seconds.
  8. Remove the device and gently massage the injection site for another 10 seconds.
  9. Look for the red plunger in the viewing window to confirm the dose was delivered.
  10. Do not attempt to recap the EpiPen. Place the used device in a sharps container or follow local disposal guidelines. If a carrying case is provided, only use it if it safely stores the device post-use.
  11. Call 9-1-1 or go to the nearest emergency room immediately. The effects of epinephrine may wear off in 15 to 20 minutes.
  12. Bring the used EpiPen with you for proper disposal and ask for a replacement prescription before leaving the hospital.

Frequently Asked Questions (FAQs) About EpiPens

1. When Should I Use an EpiPen?

At the first sign of a severe allergic reaction—even if symptoms come on suddenly or without warning. Discuss your individual action plan with your doctor.

2. Can I Use an EpiPen Through My Clothes?

Yes. EpiPens are designed to be used through clothing. Be sure the needle reaches the outer thigh muscle and hold it firmly in place.

3. How Long Should I Hold the EpiPen in Place?

Hold the device firmly in the thigh for about 10 seconds before removing it.

4. How Should I Store My EpiPen?

Store it at room temperature (68–77°F or 20–25°C), away from direct sunlight. Avoid extreme

5. Can I Travel with an EpiPen?

Yes. Keep your EpiPen with you at all times when traveling. Make sure it hasn’t expired, and store it in an easily accessible place.

6. Do I Need a Prescription?

In most countries, including the U.S., a prescription is required. Talk to your provider if you have a history of severe allergic reactions.

7. Can I Use an Expired EpiPen in an Emergency?

Expired EpiPens may not work as intended. However, in a life-threatening situation, using an expired EpiPen is better than doing nothing. Replace EpiPens before they expire—ideally at least a month in advance—and keep extras in key locations.

8. Is an EpiPen Reusable?

No. EpiPens are single-use devices. After one injection, the device should be safely discarded and replaced.

9. Can Children Use EpiPens?

Yes. Children under 30 kg (66 lbs) are typically prescribed EpiPen Jr. (0.15 mg). Parents and caregivers should ensure children are educated on how to use it, recognize symptoms, and act quickly in an emergency.

10. What If I Accidentally Inject Myself or Someone Else?

Seek immediate medical care after any accidental injection, even if allergy symptoms are not present.

11. Can I Use an EpiPen on My Pet?

No. EpiPens are for human use only. Do not administer them to animals. If your pet is experiencing an allergic reaction, contact a veterinarian immediately.
